Previous Theater Experience

Please include your prior theater experience below. We ask that provide the information as follows, using one line per show:

Date of Production (at minimum the year): Name of Show, Theater Company, Your Role, Vocal Range (if applicable)

If you already have a head shot and/or resume for your theater experience, please bring them with you to our auditions. Otherwise, during your audition, our casting team will use the information you submit here, and will take a polariod of you for casting purposes.

About Our Application

Thank you for your interst in our upcoming production! To help make your audition experience go more smoothly, please complete this form prior to the night of your audition. This will allow our production crew to review your information and have it on hand at the time of your audition.

Should you choose to not submit this application, you will be required to fill one out in hardcopy at the time of your audition.

PLEASE NOTE: In addition to submitting this form, printed theater resumes and professional headshots can be submitted at the time of your in-person audition.
